Provided policy and program planning support to the Governor’s Health Policy Coordinator, Washington State Department of Health, and Washington Health Care Authority to prepare a plan to be submitted to the Center for Medicaid and Medicare Innovations program in the context of the Affordable Care Act. Assisted the agencies in identifying the social determinants of health that affect vulnerable families, and the public and community organizations working to address them; developed a stakeholder engagement plan for relevant organizations; assisted in grant writing for the plan; and assisted in establishing Accountable Communities of Health across the state and implementing other elements of the plan. (2013 – 2017)

Results: The state won a $92 million federal grant to implement the plan, and secured a Medicaid waiver worth $1.3 billion based on the innovation plan.
